29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while accessing materials from a storage rack: (a) Inside storage shelves, on or about July 20, 2012, and at times prior thereto, employees did not have a safe access to the top shelves of the storage unit and were exposed to a fall hazard from a six foot ladder when trying to access the top shelf. (b) Inside storage shelves, on or about July 20, 2012, and at times prior thereto, employees did not have a safe access to the top shelves of the storage unit and were exposed to a fall hazard of 4-8 feet when climbing on the storage rack to access materials. Note: Abatement certification is required for this item.
