29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a. At the Ohio Packing Company facility the employer had an open sided floor approximately 10 feet above the floor below with inadequate guardrails where employees would unload materials transported from the conveyor that led from the basement.

29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. a. At the Ohio Packing Company facility located in Columbus, Ohio the employer had stairs that ran from the first floor to the basement storage, that did not have a standard railing on the open side of the top two stairs.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) At the Ohio Packing Company facility in Columbus, Ohio there was an amputation on or about July 26, 2012 on the sanitation shift while an employee working with the Rietz 3000 pound Ribbon Blender (Model RS-28-K5406, Serioal # RS-800221) reached into the discharge chute of the blender where the blades were not guarded. b) At the Ohio Packing Company facility in Columbus, Ohio the employer had a screw conveyor that was used to transport meat from the meat grinder to the Rietz 3000 pound Ribbon Blender. The screw of the conveyor was not guarded where it would catch meat falling from the grinder.
