29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) 26 Crestview Road, Mountain Lakes, NJ.    Employee was observed jumping from tubular frame scaffold onto balcony platform that was approximately 3 feet from the scaffold exposing the employee to fall approximately 24 feet to the ground.                                       Violation observed on or about 07/28/2012.    RAFF Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard which was contained in OSHA Inspection Number 312364615, Citation 1, Item Number 3, issued on 07/21/2008, with final order date 08/15/2008, with respect to a workplace located on 1004 Tabor Road, Morris Plains, NJ.    RAFF Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard which was contained in OSHA Inspection Number 313993198, Citation 1, Item 2, issued on 09/16/2010, with a final order dated 10/08/2010, with a respect to a workplace located on 5 Hapgood Court, Boonton, NJ.
