29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Northwest corner of site; On or about July 26, 2012 and at times prior thereto, an employee assisting with measuring the grade of a 4 foot deep excavation with nearly vertical walls for the installation of PVC waste water piping was not provided with a safe means of entering/exiting. The employee was observed jumping into and climbing up the excavated wall to get out of the excavation. Note: Abatement certification is NOT required for this item.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) Southeast corner to the northeast corner to the northwest corner of site; On or about July 26, 2012 and at times prior thereto, employees were setting PVC waste water piping in a 4 foot deep excavation with nearly vertical walls and the excavated material piled up at the immediate edge of the excavation. Note: Abatement certification is NOT required for this item.
