29 CFR 1926.652(a)(1): 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) a) Inside a 6.2-foot deep excavation - During the installation of a 16-inch forced main wet tap employees were working in an unprotected excavation next to an active roadway and were exposed to engulfment hazards. The soil was previously disturbed type "C" soil and the slope was 62 degrees, no protective system (shoring, shielding or sloping) was provided for employees. Observed on or about July 18, 2012.
