29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about June 26, 2012:    Employees were exposed to fall hazards of approximately 13.6 feet while utilizing a working level stacking brick materials, on a scaffold system that was not fully planked between the front uprights and guardrail posts.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. End frames were used as a means of access.    On or about June 26, 2012:    Employees were exposed to fall hazarrds of approximately 13.6 feet while accessing a scaffoold working area without the use of ladder or other acceptable means, employees were utilizing the end frames to gain access.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about June 26, 2012:    Employees were exposed to fall hazards of the following:    a) Employee standing on top section of scaffold catching and stacking brick materials was exposed to fall of approximately 13.6 feet.    b) Employees standing and working on an outrigger portion of scaffold system were exposed to falls of approximately 12.6 feet when no end rails were installed.

29 CFR 1926.451(h)(1):     Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    On or about June 26, 2012:    Employee was exposeed to struck by hazards from brick materials that were stacked upon the top of scaffold system at a height of 13.6 feet, when owner was not utilizing additional protection to protect employee.
