29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, and the employer could not demonstrate that a wider space was necessary: On July 25, 2012, inside Building 17, along the west wall, an employee was exposed to a fall of 7 feet to the ground below, while taping and floating drywall from a mobile scaffold which was not fully planked.

29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident or the in-patient hospitalization of three or more employees as a result of a work-related incident, the death was not reported by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident: On July 25, 2012, the employer failed to notify OSHA, within eight hours, of the fatality that had occurred at the workplace. OSHA was made aware of the fatality from the General Contractor approximately 16 hours after the fatality occurred.
