29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables were used:    On or about July 30, 2012, the cord to the Makita circular saw had a cut in the insulation, exposing employees to an electrical shock hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the cut in the circular saw cord is fixed.

29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    On or about July 30, 2012, the cord to the Makita circular saw had exposed wires where the cord attaches to the plug, exposing employees to an electrical shock hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the strain relief issue on the circular saw cord are fixed.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    On or about July 30, 2012, an employee was working on the top plate of a residential structure approximately nine feet and four inches high without fall protection, exposing the employee to a fall hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that fall protection is provided to and used by employees.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about July 30, 2012, an employee was working on the top plate of a residential structure at approximately nine feet and four inches high without the employer providing fall protection training, exposing the employee to a fall hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that fall protection training is provided to the employees.

29 CFR 1926.1051(a):     Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    On or about July 30, 2012, and at times prior thereto, an employee used the building frame to access the top plate of a residential structure approximately nine (9) feet and four (4) inches high, exposing the employee to a fall hazard.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees use a ladder to access anywhere on site that has an elevation change greater than nineteen (19) inches.
