29 CFR 1926.602(d): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of training and evaluation specified in 29 CFR 1910.178(l): (a) 415 Harrisburg Avenue Lancaster, PA - Employees operating the Pettibone 8044 rough terrain forklift did not receive a combination of formal instruction, practical training, and an evaluation of the operator's preformance in the work environment, on or about July 24, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

No abatement certification or documentation required.
