NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1428(a): The employer did not ensure the signal person met the Qualification Requirements (paragrapy(c) of this section) prior to giving any signals. This requirement must be met by using either Option (1) or Option (2) of this section. a) Winslow Township Municipal Building, 125 South Route 73, Hammonton, NJ 08037: On or about May 7, 2012, a crane toppled after attempting to hoist an HVAC unit. Employees signaling the crane operator were not trained nor evaluated on the Standard Method for hand signals. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
