29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:    On or about July 27, 2012, an employee was exposed to fall hazards of approximately 14 feet when the employee was standing on a job-made scaffold that used a ladder placed horizontally over boards that had been nailed to the house wall and roof as the working platform.  The ladder was not planked.  There were no guardrails.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    On or about July 27, 2012, an employee was exposed to fall hazards of approximately 14 feet when the employee was standing on a job-made scaffold that used a ladder placed horizontally over boards that had been nailed to the house wall and roof as the working platform.  The ladder was not fully planked.  The single board on the ladder did not cover the entire width of the ladder and was only approximately 3-4 feet long.

The ladders had a shallow incline of approximately one-half due to the need to lean against the house and clear the small roof over the front door.
