29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. South Outside Building 4 - On or about July 27, 2012 employee used pump jack scaffold at a height of 18 feet to install vinyl soffit with no end netting to prevent falling from the scaffolding.

29 CFR 1926.1052(c)(1)(ii): One stairrail system along each unprotected side or edge. Note: When the top edge of a stairrail system also serves as a handrail, paragraph (c)(7) of this section applies. Building 5 - On or about July 27, 2012 employees were exposed to fall hazards while using the second and third sections of wooden stairways that did not have stairrails. The employees used the stairs to carry materials to the roof. The top step on the second level is 9 feet 07 inches and the top step on the third level is 14 feet 06 inches to the ground below.
