29 CFR 1910.23(a)(8):     Every floor hole into which persons can accidentally walk was not guarded:    On or about 5/1/12, the areas between the shelves of the warehouse racks were covered by boards that were not capable of supporting the weight of an employee.  Employees were not prevented from walking in these areas.
