29 CFR 1926.403(b)(2):     Installation and use. Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.    a) On or about July 30, 2012; at the first floor; the employees were using power strips/taps to provide power to portable equipment such as but not limited to sprayers, fans and the battery charging stations; either directly or through portable extension cords. The power strips/taps are not designed for outdoor or construction site use.

29 CFR 1926.404(b)(1)(ii):     Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.    a) On or about July 30, 2012; at the first floor; the employees were using power tools such as but not limited to sprayers and saws that were plugged in directly or by portable extension cords into outlets that were not equipped with a ground fault circuit interrupter.

29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    a) On or about July 30, 2012; at the first floor; the employees were using portable extension cord which had the ground pin missing. The extension cord was being used to power the sprayer used in the process.

29 CFR 1926.405(b)(2):     Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a) On or about July 30, 2012; at the first floor; the employees were using power tools such as but not limited to sprayers and saws that were plugged in directly or by portable extension cords into outlets that were not properly guarded with a face plate.
