29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).    On or about July 26, 2012, employees were exposed to fall hazards of approximately 14-20 feet when the employees were working on a scaffold that was not fully planked on any level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      On or about July 26, 2012, employees were exposed to fall hazards of approximately 14-20 feet when the employees were working on a scaffold that had no ladder.  Employees accessed the second and third levels by climbing up/down the scaffold frames.

On or about July 26, 2012, employees were exposed to fall hazards of approximately 14-20 feet when the employees were working on a scaffold that was missing guardrails on the front of the scaffold on the second level, on the third level where no scaffold frames had been installed, on the ends of the third level, and on the ends of the side-brackets.
