29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) GF Roofing, Taft Farms, Loveland, CO 80538: On and before July 30, 2012, the employer did not ensure that employees were protected from injury, in that employees were performing trim, gutter work, and laying shingles on the roof of a residential home without utilizing a fall protection system. This condition exposed employees to fall hazards of over 11 feet.

29 CFR 1926.502(d)(16)(ii): The employer did not ensure that personal fall arrest systems, when stopping a fall, were rigged such that an employee could neither free fall more than 6 feet, nor contact any lower level: (a) GF Roofing, Taft Farms, Loveland, CO 80538: On and before July 30, 2012, the employer did not ensure that employees were adjusting the rope length on their life lines to prevent them from free falling no more than 6 feet, nor contact the ground. This condition exposed employees to fall hazards of over 11 feet.
