29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.    a)  Exterior walls of residential home, scaffold:  The employer exposed employees to fall hazards while working over 10 feet above the ground from a fabricated frame scaffold with platforms that were not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a)  Exterior walls of residential home, scaffold:  The employer exposed employees to fall hazards of over 10 feet as a secure ladder was not used by employees to access the scaffolding platform where work was being performed.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a)  Exterior walls of residential home, scaffold: The employer exposed employees to fall hazards while working over 10 feet above the ground from a fabricated frame scaffold which did not have a complete guardrail system, due to the mid-rails, cross-bracing, and end-rails not being installed.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  Training and Documentation:  The employer failed to instruct and train employees on the hazards associated with and fall protection requirements while exposed to heights above 10 feet while on a tubular frame scaffold.
