29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Facility, 3110 6th Avenue, Altoona, PA: On or before 07/30/12, an employee was standing on the 1st step of a Reddy-Lock step-ladder located on a  4 foot x 4 foot wood pallet, supported by the front fork attachment of a IR Bobcat Skid Steer S185, scrapping paint. The employee was exposed to an 11 foot 3 inch fall hazard to the ground level.

29 CFR 1926.451(c)(2)(iv):     Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a) Facility, 3110 6th Avenue, Altoona, PA: On or before 07/30/12, an IR Bobcat Skid Steer S185 was used to support a 4 foot x 4 foot wood pallet which was used as a temporary work platform by an employee to scrap paint. The employee was exposed to an 11 foot 3 inch fall hazard to the ground level.

29 CFR 1926.451(f)(15):     Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.    Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways thrust exerted by the ladder.    a) Facility, 3110 6th Avenue, Altoona, PA: On or before 07/30/12, an employee was standing on the 1st step of a 4 foot Reddy-Lock Alumiladder step-ladder located on a 4 foot x 4 foot wood pallet, supported by the front fork attachment of a IR Bobcat S185. The employee was using the ladder to increase his overall working height to approximately 11 feet 3 inches from the ground level to scrap paint on the wall of a building.
