Premier Roofing Design, Inc. had employees exposed to fall hazards and struck-by hazards on the head, on a commercial construction site in Champaign, Illinois. The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.

Premier Roofing Design, Inc. had employees exposed to fall hazards and struck-by hazards on the head, on a commercial construction site in Champaign, Illinois. The employer did not have a competent person at the construction site, who was competent in safety rules and requirements, in order to conduct frequent and periodic safety inspections at the construction site.

Premier Roofing Design, Inc. had an employees exposed to struck-by hazards on the head without head protection as work was done above him and loads were passed over his head, on a commercial construction site in Champaign, Illinois. The employer did not provide employee training in order to provide the employees with the knowledge to recognize construction safety hazards, as well as the procedures to follow in order to minimize or eliminate those safety hazards.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. Premier Roofing Design, Inc. had employees exposed to 10 ft., 7 in. fall hazards without fall protection, on a commercial construction site in Champaign, Illinois.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Premier Roofing Design, Inc. had employees exposed to 10 ft., 7 in. fall hazards without fall protection, on a commercial construction site in Champaign, Illinois. The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards of falling and the safety requirements associated with fall protection, as well as the proper work procedures to follow, in order to minimize and eliminate the fall hazards.
