29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. a) Jobsite, 25th Street and 8th Avenue, Altoona, PA: On or before 07/31/12, an employee was operating a JLG aerial lift, Model 800S, SN0300136893, without a personal fall arrest system attached to the boom or basket. The employee was exposed to a fall hazard of approximately 24 feet to the ground level.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: a) Jobsite, 25th Street and 8th Avenue, Altoona, PA: On or before 07/31/12, a Buckeye portable fire extinguisher, 21C-876079, was not charged. The portable fire extinguisher was located on a JLG aerial lift, Model 800S, being used by an employee to perform painting on the building.
