29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANS:    (a) On the south side of the building, the company owner was repairing a parking lot light pole light fixture from a shop-made metal frame work platform that was attached to an Altec AT250-G aerial lift truck. The work platform was not certified by the manufacturer or a nationally recognized laboratory to ensure conformity with applicable provisions, exposing the worker to a fall hazard of approximately 27 feet to the next lower level.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) On the south side of the building, the company owner was exposed to a fall hazard while repairing a parking lot light pole light fixture from an Altec aerial lift, approximately 27 feet above the next lower level, without wearing a restrain or positioning device.
