Kennametal Inc. - On or about 7/26/12, separate OSHA 300 Injury and Illness forms were not maintained for Northgate Circle and Frienz Drive sites. The injuries and illnesses for both sites were being maintained on the same OSHA 300 forms.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected: A.) Kennametal Inc., Frienz Drive - On or about 7/26/12, the periodic inspection only included a physical walk-around review of energy isolation points. The periodic inspection did not audit employees and/or review the authorized employees responsibilities under the energy control procedures. Employees performed servicing and maintenance work on machines/equipment that included, but was not limited to the Water Jet Cutter UWJ #1 and the VIm Furnace UPVT #3.
