Condition exposed this employee to the hazard of eye injuries.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Four Seasons Heating, Inc. at 8099 E. 32nd Ave., Denver, CO 80238: On or before July 21, 2012, the employer allowed an employee to use an orange extension cord that was missing its ground plug, for use in powering an 110 volt light, model THD WL5005D, in the basement. Condition exposed this employee to an electrical shock hazard.
