29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system: (a) On or about June 7, 2012, at a worksite located at 10th Street and Burlington in North Kansas City, MO, employees engaged in removing the roof from an existing structure were exposed to fall hazards of greater than 6 feet while working in close proximity to the edges of the roof and holes formed by the removal of the existing roof. Although the employees were wearing fall harnesses, and most had lanyards, their fall protection equipment was not connected to any type of lifeline or other suitable anchorage point.
