Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system: a) Jobsite at 115 Arnold Palmer Drive, Latrobe, PA: On or about July 31, 2012, an employee was working from an aerial lift and was not wearing a safety harness with a lanyard tied off in the boom or basket.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Jobsite at 115 Arnold Palmer Drive, Latrobe, PA: On or about July 31, 2012, an employee was working on a roof and was not protected from falling. The fall hazard to the employee was approximately 11.5 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not be secured at its top to a rigid support that would deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite at 115 Arnold Palmer Drive, Latrobe, PA: On or about July 31, 2012, an employee was using a 20 foot aluminum extension ladder to access the canopy roof and the ladder was extended approximately 28-30 inches above the roof he was accessing. The fall hazard to the employee was approximately 11.5 feet to the ground below.
