29 CFR 1910.133(a)(3):  The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.      (a)	BC Granite & Marble, LLC, 1874 E. Harmony Rd., #8, Ft. Collins, CO 80528: On and before August 14, 2012 an employee was wearing his personal prescription glasses without wearing eye protection over the prescription glasses. The employee conducts granite countertop fabrication operations.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").    Note: 	In lieu of wearing protection over prescription glasses, the employer can provide the employee with prescription safety glasses.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    (a)	BC Granite & Marble, LLC, 1874 E. Harmony Rd., #8, Ft. Collins, CO 80528: On and before August 1, 2012, the employer did not ensure that the employees were trained in the safe operation of powered industrial trucks, in that operators were not trained to use the Nissan forklift.  This condition exposed the employee to the hazard of improper forklift operation.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

Flammables were stored in a Greenlee 3660 which is not rated for flammable storage.
