29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    On or about July 25, 2012, employees were exposed to fall hazards of approximately 17.4 feet when employees were working on steep roofs on buildings 1515 and 1523 without fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about July 25, 2012, employees were exposed to fall hazards of approximately 17.4 feet when employees working on building 1515 used a ladder which did not extend three feet above the edge of the roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.    On or about July 25, 2012, employees were exposed to fall hazards of approximately 17.4 feet when employees had not been trained to recognize ladder fall hazards such as, but not limited to, ladders that did not extend at least three feet above the edge of the roof.
