(b) Cell 400; The welding employee was wearing a makeshift positive air purified respirator which was not NIOSH-certified.

29 CFR 1910.1026(d)(2)(iii): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be at or above the action level; however, the employer did not perform periodic monitoring at least every six months:    (a) Cell 100 and 200; Air sampling conducted on 3/16/2010 by an outside firm indicated hexavalent chromium levels to be at or above the action level; no further air monitoring was conducted.     (b) Cell 100; Air sampling conducted on 3/18/2011 by an outside firm indicated hexavalent chromium levels to be at or above the action level; no further air monitoring was conducted.
