The employer did not provide a swinging gate or so offset that a person could not walk directly into the opening for the following but not limited to locations;    1. 29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    Location A: 68 Central Street, Acton, MA and Location B: 147 Lowell Road, Concord, MA    On or about 7/26/12, employees were exposed to fall hazards of up to 25 feet while walking and working on top of the oil storage tank platforms to check the inventory levels on top of the tanks.  The employer did not provide mid-rails and toe boards on the platform s for the following but not limited to locations;    1. Instance 1: Location A - 68 Central Street, Acton, MA - Tank 7&8 work platform.  A section of standard railing was missing at the end of the platform.  2. Instance 2: Location A - 68 Central Street, Acton, MA - Tank 1&2 work platform  3. Instance 3: Location B -  147 Lowell Road, Concord, MA - Tanks 1, 2, 3, and 4 work platform  4. Instance 4: Location B -  147 Lowell Road, Concord, MA  Tank 6 work platform    Location B: 147 Lowell Road, Concord, MA  Tank 5 work platform    Instance 5: On or about 7/26/12, employees were exposed to fall hazards of 25-feet while walking and working on top of oil storage tank #5 platform to check the inventory levels on top of the tanks.  The employer did not guard the top of the tank with standard guard railing, mid-rails, and toe boards.    Location A: 68 Central Street, Acton, MA and Location B: 147 Lowell Road, Concord, MA    Instance 6: On or about 7/26/12, employees were exposed to fall hazards of 8 feet, 5.25-inches while walking and working on top of the oil truck that was not guarded by standard railing.
