29 CFR 1926.305(c): Blocking. When it is necessary to provide a firm foundation, the base of the jack shall be blocked or cribbed. Where there is a possibility of slippage of the metal cap of the jack, a wood block shall be placed between the cap and the load.    a.  On or about July 25, 2012, at the above addressed jobsite, employees used a hydrolic car jack to support a roof from collapsing, the employer failed to ensure the jack was blocked to prevent slippage.  Employees were thereby exposed to the hazards associated with collapsing structures.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a.  On or about July 25, 2012, at the above addressed jobsite, employees performing residential carpentry operations on a low sloped roof (4:12 or less), twelve  (12) feet eleven (11) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:    a. On or about July 25, 2012, at the above addressed jobsite, employees conducting carpentry operations from a low slope roof, twelve (12) feet eleven (11) inches above the ground, secured life line ropes to a limb of a silver maple tree, thereby exposing the employees to the hazards associated with falls.
