29 CFR 1926.501(b)(3):  Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:    a)  Employees were exposed to the hazard of falling approximately 30 feet to the ground while working in the hoist area.    No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:    a)  Employer did not provide training to each employee in order to recognize hazards associated with working at an elevation and to understand procedures to control or minimize those hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
