29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. On August 2, 2012 employee(s) were working on a roof installing siding without any means of fall protection. The employee was exposed to a fall hazard of approximately 9.2 feet. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On August 2, 2012 employees were using a ladder to access the roof. The ladder did not extend three feet above the landing surface exposing employees to a fall hazard. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
