29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Receiving Area, north end, battery charging station: the Emergency Eye Wash Station available for use was a Drench Hose. It was not adequate for an eyewash station in that it did not meet the requirements of being designed to be used without the use of the operators hands and shall provide flushing fluid to both eyes simultaneously.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: Raytheon Room, northwest section: The rotating shaft drive for the conveyor belt guard was not fully on. It had been damaged and one bolt holding it in place, allowing the rotating shaft to be unguarded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Production Room #3: On the 240 volt yellow power cord in the center area, an electrical power drop from the overhead area, the outer insulation was pulled away from the electrical plug, allowing strain to be placed on the inner electrical connections. b) Main Processing Room, south end: On the 110 volt flexible cord for the 48 inch Dayton Box Fan, the outer insulation was pulled away from the electrical plug, allowing strain to be placed on the inner electrical connections.

29 CFR 1910.305(j)(2)(i): All 15- and 20-ampere attachment plugs and connectors were not constructed so that there were no exposed current-carrying parts except the prongs, blades, or pins: Grinding Room, north wall, a 110 volt electrical receptacle was damaged, a section of the receptacle box was broken, exposing the inner electrical wiring.
