29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a. On or about July 31, 2012, Temporary Electric Panel of Building, A yellow extension cord lacked strain relief in that the outer jacket was pulled away from the attachment plug exposing the covered conductors. The extension cord was plugged into a temporary receptacle outlet which exposes employees to electrical hazards.
