29 CFR 1926.501(b)(10): Employee(s) engaged in roofing activities on low-sope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) North end of Mitchell Hall at Nichols School, no fall protection was provided for the five (5) employees observed doing roofing work exposed to falls of 27 feet, 3 inches to the ground below. b) West side of Mitchell Hall at Nichols School, no fall protection was being utilized by the three (3) employees doing roofing work and were exposed to falls of 32 feet, 6 inches to the blacktop/concrete surface below. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) West side of Mitchell Hall at Nichols School, employee observed climbing a tower of frames consisting of tubular welded frame scaffolding and no ladder was provided. The initial step from the ground to the cross member of the frame was 29 1/2 inches and it was 19 1/2 inches between each cross member on the frames of the scaffold. The employee was exposed to falls up to 32 feet, 8 inches to the blacktop/concrete below. ABATEMENT CERTIFICATION REQUIRED
