29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. South side of building, scaffold: The employer exposed employees to fall hazards of 12 feet to the ground as a secure ladder was not used by employees to access the scaffolding platform where work was being performed. The employees were replacing and painting panels for the commercial building awning.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. South side of the building, scaffold: The employer exposed employees to fall hazards while working over 12 feet above the ground from a ladder jack scaffold which did not have a complete guardrail system, due to the mid-rails, top-rails, and end-rails not being installed. The employees were in the process of replacing and painting panels for the commercial building awning.
