OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed and or struck by a falling load.    A)  In the workshop, on or about August 1, 2012, and at times prior to, employees operated crane #3 (20 TON) with  electric hoist to lift steel axle approximately 5ft. long and 2000 lbs.  The hook used with the hoist did not have a self- closing spring operated latch across the throat of the hook to prevent the steel axle from accidentally becoming detached from the hook, exposing employees to struck-by falling material.    Among other methods, feasible and acceptable methods to correct this hazard included but are not limited to:    1.  Conduct frequent and periodic inspections of the lifting device in accordance with ASME B.30.10.  Ensure that employees inspect to see that the safety latches for the hooks are in place and functioning and to use the type of hooks appropriate for the type of work that are being performed.      ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.178(a)(4):     Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    A)  In the workshop, on or about August 1, 2012, and at times prior to, employees operated forklifts with Serial numbers AT29C80313, AT29C00834 and AT19C50718 attached with personnel platform to elevate employees without manufacturer's prior written approval, exposing employees to fall hazard.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.179(j)(2)(iv):     Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected.    A)  In the workshop, on or about August 1, 2012, and at times prior to, chain safety inspection tags were missing on crane #1(Kone 10Ton) and crane #3(20 Ton).  Employees operated them to lift steel axle weighing about 2000 lbs. each exposing them to struck-by falling material.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.252(b)(2)(iii):     Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.    A)  In the workshop, on or about August 1, 2012, and at times prior to, employees were observed welding without fllash curtains or screens, exposing employees to ultra violet rays.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.
