29 CFR 1910.141(a)(5):     Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin:  (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) (DESCRIBE HAZARD(S) WHERE NECESSARY)    a) In the rear of the restaurant, there were several rat (mice) droppings (feces) located inside of the metal muffin pans. The muffin pans were located on a metal shelf in the rear of the facility.

29 CFR 1910.157(c)(1):     Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND CONDITION(S))(DESCRIBE HAZARD(S))  NOTE: APPLIES WHEREVER EXTINGUISHERS ARE PROVIDED FOR EMPLOYEE USE INCLUDING EXTINGUISHERS ON THE OUTSIDE OF WORKPLACE BUILDINGS OR STRUCTURES    a) In the middle of the back room, there was a fire extinguisher located on the floor underneath a sink exposed to physical contact. The fire extinguisher could be kicked when employees are washing their hands.

29 CFR 1910.303(g)(1):     Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    a) In the rear of the restaurant, near the back exit door, there were two (2) blocked 110/220 volt panel boxes. Panel boxes (A and A1) were blocked by a large freezer.

29 CFR 1910.305(g)(1)(iv)(A):     Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)(LOCATION)(IDENTIFY EQUIPMENT AND CONDITION(S))(STATE HAZARD(S) TO EMPLOYEE(S))    a) In the rear of the restaurant, there was a white extension cord plugged into another extension cord on the east wall and this extension cord was plugged into a powerstrip located near the west wall. This fifty (50) feet of extension (flexible) cords was being used to energize the large freezer.
