29 CFR 1926.1053(b)(15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. On or about August 1, 2012, employees used a job made ladder that had not been inspected prior to use and were exposed to a fall of more than 13 feet to the concrete below.
