29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. On or about 07/31/2012, the employer did not ensure that a wire rope sling used to lift steel beams, weighing approx 600 lbs, was inspected prior to or during use to ensure that the sling was free of defects. Wire rope sling ID # 23658610, was permanently kinked, end eyes were birdcaging and the individual wire strands had been flatened at the eyes, reducing the capacity of the sling.
