29 CFR 1926.501(b)(4)(i) : Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) In the front lobby on or about July 31, 2012, there was no fall protection for employees for about 24 feet along the edge of a hole for a stairwell. It was about 8 feet from the edge of the hole to the stairway landing below, and it was about 16 feet to the lowest level runner of the open pan stairs. The employer removed guardrails that clamped onto the edge of the hole earlier that morning to install steel studs for a wall in the stairwell below the lobby floor. A single horizontal ribbon of barrier tape was placed about 3 feet above the lobby floor and about 1 to 2 feet from the edge of the stairwell hole. Employees accessed the building through the lobby area that was next to the stairwell hole.
