29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:      (a) In the front lobby area on or about July 31, 2012, there was no midrail or equivalent between the surface of the concrete runway over the lobby and the toprail.  The cleaning crew removed the midrail of the clamp on guardrail system earlier in the day to remove and place debris in dumpsters.  Employees used the concrete runway to access the different sections of the hotel.  It was about 13 feet from the runway to the concrete lobby floor below.
