29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. Jobsite: Approximately nine second floor window openings were unguarded exposing employees to a fall hazard of approximately 15 feet.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. Jobsite: The orange electrical extension cord ran through a large puddle. The extension cord was used to provide power to the circular saw used on the second floor. The extension cord's receptacle lacked a ground fault circuit interrupter.
