Thompson rivet machines were not guarded to prevent contact with the mechanisms that powered the rivet presses (rotation and displacement). b) Plant, Compression Molding Area - On or about 06/26/12, the Lawtomatic press #3 (S.N. 15083, 36 in. max. stroke) was not guarded to prevent crush hazards during the displacement (descent) of the press against the pedestal. c) Plant, Rotational Molding area - On or about 06/26/12, the 12 ft. open areas in front of the rotational molding machines #1 and #2 were not guarded to protect operators from accidental contact (crush) between the rotating mold and the floor. 29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) Plant, Tray Area - On or about 06/26/12, the flexible power cable (240 V) in the back of the Verson press bender #2 S.N. 6393-2062 had a damaged outer jacket, and a section was covered with electrical tape. b) Plant, Tray Area - On or about 06/26/12, a flexible metallic conduit in the back of the Verson press bender #2 (S.N. 6393-2062) was loose and withdrawn from the junction box. The internal conductors were exposed. c) Plant, Compression Molding Area - On or about 06/26/12, the 240 V flexible power cable for the heating elements in the Lawtomatic press #3 (S.N. 15083, 36 in. max. stroke) had a broken male plug. d) Compression Molding Area - On or about 06/26/12, the junction box for the heating elements of press #3 (S.N. 15083) was not closed. The internal conductors and splices were exposed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
