29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    The employer does not protect employees from falling when working 6 feet or more above lower levels. This violation was observed on or about August 1, 2012, on the steep roof of a newly built bank where employees were exposed to a 14 foot fall hazard when performing roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer does not ensure that portable ladders used for access to an upper landing are extended at least 3 feet above the upper landing surface accessed. This violation was observed on or about August 1, 2012, on the roof of a newly built bank where employees were exposed to an approximately 14 foot fall hazard while using a metal extension to access the roof and the ladder was extended approximately 18 inches above the roof.
