29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: The employer does not ensure that personal protective equipment is worn when operating an excavator. This violation was observed on or about August 2, 2012, on FM 2920 Road where employees were exposed to an ejection hazard when operating a Bobcat 311 excavator without donning the seatbelt.
