29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Sundance Roofing, 4321 W. Bellewood Drive, Denver, CO 80123: On and before August 2, 2012, the employer did not ensure that employees were protected from injury, in that employees were performing re-roof work and modified torch work on the fron and back side of a residential roof without utilizing a fall protection system. This condition exposed employees to fall hazards of over 7 feet.
