29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. On or about 08/02/2012, the employer did not ensure that open holes in the floor were covered or otherwise protected. Open holes were observed in the slab at the back of the structure at the locations listed below, and varied in size. All were approx 12-18 inches deep and were in areas heavily travelled by employees. INSTANCES: a. Two holes in the food prep area b. One hole at the door to the freezer area c. One large hole at the future location of the ice machine
