29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Ace's Roofing, 4321 W. Bellewood Drive, Denver, CO 80123: On and before August 2, 2012, the employer did not ensure that employees were protected from injury, in that employees were performing re-roof work and modified torch work on the front and back side of a residential roof without utilizing a fall protection system. This condition exposed employees to fall hazards of over 7 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Ace's Roofing, 4321 W. Bellewood Drive, Denver, CO 80123: On August 2, 2012, the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities worked near the edge of a low pitch, residential home without the use of a fall protection system. This condition exposed employees to a fall in excess of 7 feet.
