29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety nets systems, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Irineo Osorio at 2680 B Road, Grand Junction, CO 81503: On 7/24/12, the employer did not ensure an employee engaged in construction activities was protected from injury, in that an employee was engaged in roofing activities without the use of fall protection. This condition exposed the employees to a fall hazard of approximately 8 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder: (a) Irineo Osorio at 2680 B Road, Grand Junction, CO 81503: On 7/24/12, the employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employees were using a portable ladder for access to the roof. Employees were exposed to a fall of approximately 8 feet while attempting to use the ladder. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

Employees were using the top step of the ladder to access the roof. Employees were exposed to a fall of approximately 8 feet while attempting to use the ladder.
